Why estimates fail

Most calorie targets and heart-rate zones in circulation are built from population formulas: plug in age, weight and sex and the calculator returns a number that applies, on average, to people who share those characteristics. The problem is that "on average" is not the same as "you." Studies comparing measured resting metabolic rate against predictive equations consistently find meaningful individual variation, and the size of that gap tends to be larger in athletic populations, exactly the people most likely to be training hard enough for the error to matter (comparison of measured and predicted resting metabolic rate in adolescent athletes and non-athletes, PMC). A calorie target built on a formula that is off by a few hundred calories a day compounds into a materially wrong nutrition plan over weeks. The same logic applies to heart-rate zones set from an age-based formula rather than an individual's actual gas-exchange data.

The consequence of relying on an estimate is not academic. A member training toward a fat-loss goal on a calorie target that overstates their true resting metabolic rate can spend months in what should be a deficit but is not, and never understand why the plan is not working. A member training toward a performance goal using textbook age-based heart-rate zones can spend an entire block training at the wrong intensity for their own physiology, undermining exactly the aerobic or threshold adaptation they were trying to build. Measurement removes that ambiguity at the source, which is why it is the first step in several of our programs rather than an optional upgrade.

What we measure, and on what equipment

Metabolic testing at Aeon runs on the KORR CardioCoach™, an in-house indirect calorimetry system. The resting portion measures the oxygen your body consumes and carbon dioxide it produces at rest, from which resting metabolic rate is calculated directly rather than estimated. The exercise portion, run on a graded protocol, measures VO₂ max — your body's ceiling for oxygen uptake during maximal effort — and identifies the substrate crossover point, the intensity at which your body's fuel mix shifts from predominantly fat oxidation toward predominantly carbohydrate oxidation. That crossover point, along with ventilatory threshold data from the same test, is what we use to set real aerobic and threshold training zones, rather than the generic 220-minus-age estimate still printed on most cardio equipment.

Why VO₂ max is worth measuring

VO₂ max is not just a performance number for competitive athletes; it is one of the more consistently studied markers associated with long-term health outcomes. In a study of 122,007 adults undergoing exercise treadmill testing at a single academic medical center, with a median follow-up of 8.4 years, patients in the highest cardiorespiratory fitness category (at or above the 97.7th percentile for age and sex) had substantially lower risk-adjusted all-cause mortality than those in the lowest fitness category, with an adjusted hazard ratio of 0.20 (Mandsager et al., JAMA Network Open). This is an association from an observational cohort, not a controlled trial of training producing longevity, and it does not mean any individual member will see a specific health outcome from improving their number. What it does mean is that VO₂ max is a marker worth knowing and worth tracking over time, not a vanity statistic.

The same study found a graded relationship rather than a simple threshold effect: risk-adjusted mortality dropped in a stepwise fashion moving from low to below-average to above-average to high to elite fitness categories, with no sign of diminishing returns even at the very highest fitness levels measured. That pattern is part of why we treat VO₂ max as a number worth retesting rather than measuring once and filing away.

From data to a training plan

A metabolic test on its own is just information. The value comes from what happens next: your personal training program is built using your actual training zones rather than an estimate, and your nutrition coaching uses your measured resting metabolic rate as the foundation for energy and protein targets. For members on a medical weight loss program, a measured resting metabolic rate at baseline, and again at intervals through the program, is one of the clearest ways to see whether a nutrition plan is calibrated correctly as body composition changes.

The substrate crossover point deserves particular attention because it is the number most estimate-based training plans skip entirely. Knowing the intensity at which your body shifts from predominantly fat oxidation to predominantly carbohydrate oxidation lets a coach build a genuinely graduated aerobic base rather than a single generic "zone 2" prescription borrowed from someone else's physiology. For members focused on longer-duration performance or on building a larger aerobic engine before adding intensity, this is often the single most useful number the test produces.

How to prepare, and how often to retest

Arrive hydrated, avoid a heavy meal in the hours before your appointment, and skip intense training the day of the test, since acute fatigue or dehydration can shift both resting and exercise readings. Most members retest on a three-to-six-month cadence, or sooner after a substantial change in training volume, a body composition shift confirmed by a body composition scan, or a change in a medical weight-loss plan, since both resting metabolic rate and fitness level change as training and composition change. A single test is a baseline; retesting is what turns it into a trend.
